I can help with all of the above, and then some. Physical therapy is not just working on range of motion or watching me type away behind a computer while you sit on a bike for 15 minutes. If you have needs related to pain or movement, you need a physical therapist.

Being a cash-based clinic means that YOU are in charge of your treatment—not insurance companies.
This means that you will receive the individualized, one-on-one treatment that you deserve without limitations to your evaluation, treatment, or time.
With same-day availability, you can start feeling and moving better without delay.

I offer a range of services designed to meet your needs—whether you're recovering from an injury, trying to get back to doing what you love pain-free, or are looking for the best way to be proactive with your physical health to prevent problems down the road. Everything is done collaboratively to help you take control of your care. The treatment modalities include but are not limited to:

We start with an initial evaluation designed to obtain a comprehensive understanding of your situation—this includes health history, thorough physical assessment, early treatment, and a collaborative discussion with regards to a plan moving forward.
Treatment sessions can then be scheduled individually, packaged sessions may be purchased, or perhaps you want to try some things on your own before committing to continued care.
You will never be pressured to buy into something you don’t believe in or cannot comfortably afford.
All appointments are 60 minutes unless you are interested in just dry needling, which are 30-minute appointments and may be scheduled following an initial evaluation.

If you’re ready to move forward with care, simply book an appointment and then fill out the intake form that is emailed to you prior to your visit. Your first visit is an initial evaluation where I will take the time go over your history, conduct a thorough assessment, and get started on treatment.

I do not accept insurance because I believe that you should be in charge of your care, not a third-party payer.
Insurance companies dictate how professionals perform evaluations, the treatments they are able to provide, the time allowed for each treatment, the length of each visit, the number of visits you get, and documentation requirements that take the time and attention off of the patient and onto the computer, all in the name of profit.
They do not consider current research or recommended best practices, and they certainly do not consider you, the patient. Insurance is a reactive entity that does not value prevention; they simply wait until it is too late and then cover the bare minimum, resulting in poor treatment from overworked clinicians buried in paperwork, double-booked schedules, and shortened sessions.

The answer to that question depends entirely on what you are looking for. My pricing will always be transparent, and you will only ever pay for the services you receive.
A discovery call is always a great free first step.

Compared to most insurance plans, services provided here will often be more affordable when considering required copayments and coinsurance. Additionally, you have more control and are not stuck choosing from the same in-network providers offering subpar services.
I will always maintain fee transparency, and you will never be surprised with added costs.
Upon request, a superbill can be provided for you to facilitate possible out-of-network reimbursement with your health insurance provider.

Due to legal restraints, Medicare beneficiaries are the one group that I cannot charge for physical therapy services out-of-pocket.
However, if you are on Medicare and are interested in my services, please reach out to discuss other options.

Polar Physical Therapy is a cash-based clinic where payments are collected at time of service.
Cash, checks, HSA/FSA cards, and all major credit and debit cards are accepted.

Every hour on the hour—see current availability
Monday through Friday—6:00 AM to 6:00 PM
Every other Saturday—8:00 AM to 12:00 PM
Additional times may be available upon request.

No!
Although I am located inside Evolve Fitness, you do not have to be a member to work with me. My arrangement with the gym allows us to utilize their equipment during our sessions regardless of membership.
If you want to use the gym outside of our scheduled time, you will need to purchase a membership.

If you would prefer to receive treatment from the comfort of your home, that can be arranged at no additional cost—just keep in mind the limitations to available equipment.
